Information collected automatically

When you visit the Site, we and our service providers may collect your IP address, browser and device type, pages visited, referring website, and time spent on pages, using cookies and similar technologies such as Google Analytics and advertising or tracking pixels.

Protected health information

Health information you submit through our secure patient forms or booking system is protected health information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA). We handle PHI as described in our Notice of Privacy Practices and use only vendors who have signed Business Associate Agreements with us.

Please do not send health information through our general contact form or by regular email. Those channels are not designed for sensitive health information. Use our secure patient forms or call our office instead.

We do not use analytics or tracking tools on pages where patient health information is entered, and we do not share PHI with advertising platforms.

If you believe a child has submitted information to us directly, please contact us and we will delete it.
